• Medallion Guarantee on the transfer form, with the following supporting documents:

• If a Medallion Guarantee is not provided, the following documents are required:
  • Original Certificate(s) or book entry statement(s)
  • Signature guarantee stamp on the transfer form
  • Corporate Resolution
    • Bank certified copy or original
    • Dated within the last six months
    • Bear the same company / corporation name as reflected in the registry on the bond(s) certificate or book entry statement
    • List of names and titles of individuals authorized to sign on behalf of the Company / Corporation
    • Document must be se signed by the Secretary or an Assistant Secretary, unless the authority to transfer is given to the signing officer, in which case the certification should be signed by another offices.
    • Provide specimen of the Officers
    • Be under corporate seal (if applicable)
